What to bring

- Any splint, brace, or other medical devices you may be using for your injury
- List of all medications (over-the-counter, vitamin/herb, and prescription) you take, including doses and how often you take them
- X-rays, MRIs, and any other imaging (not older than six months) related to your injury or condition
- Clinic notes or operative reports from your referring physician related to your injury or condition
