Before your appointment
- Sign up for our patient portal
- Fill out your paperwork (pdf) or plan to arrive 15 minutes before your scheduled appointment to allow time for paperwork
What to bring
- Insurance card (many insurance providers require you to have a referral or authorization from your primary care physician before treatment by a specialist; check with your insurance provider if you have any questions about your plan or coverage)
- Government-issued photo ID
- Any splint, brace, or other medical devices you may be using for your injury
- List of all medications (over-the-counter, vitamin/herb, and prescription) you take, including doses and how often you take them
- X-rays, MRIs, and any other imaging (not older than six months) related to your injury or condition
- Clinic notes or operative reports from your referring physician related to your injury or condition
